You do not need any login details and you can submit any medical query at a time when it suits you.

Start by choosing the service you need; e.g. an appointment request, sick/fit notes or blood or scan results.
Fill in your information
Give details of your request.
Contact from your GP
Depending upon your medical need we will contact you to arrange an appointment or provide you with advice by text or phone.

Please note that we have now published our IT Statement of Intent in line with the new GP contract.
This service is available 24 hours a day, 365 days a year service to Custom House patients which conveniently allows access to healthcare services online.
In the first instance you will require login details. Please attend the practice in person and one of our reception staff will be pleased to issue you with these. This will enable you to set up an account to use this service.

Booking Appointments for our GPs and Practice nurses
This service is designed to book appointments with your usual registered GP to ensure continuity of care as well as giving you the option of additional GP appointments if you require.
There are also appointments available with our highly trained and experienced nursing team who can assist our patients with a variety of problems, including minor illnesses, wound care, chronic disease management, care plans as well as contraceptive and travel advice.
